Telehealth Services Policy – Invigor8 Medical Care

Invigor8 Medical Care provides telehealth and wellness services through secure electronic communication technologies that allow patients to consult with licensed healthcare providers remotely.

By using our services, you acknowledge and agree to the terms outlined in this Telehealth Services Policy.

Nature of Telehealth Services

Telehealth services may include medical consultations, health evaluations, treatment planning, prescription management, wellness counseling, follow-up care, and patient education delivered remotely through approved electronic communication methods. These services may be conducted through video conferencing, secure messaging, electronic forms, telephone communication, or other authorized telehealth technologies.

Provider Evaluation and Clinical Decision-Making

All medical evaluations, treatment recommendations, and prescribing decisions are performed solely by licensed healthcare providers using available patient information, medical history, intake documentation, submitted materials, and professional clinical judgment. Treatment eligibility, prescription approval, and continuation of care are

determined exclusively by the evaluating provider. Submission of payment, medical intake forms, photographs, questionnaires, or consultation requests does not guarantee diagnosis, prescription approval, treatment eligibility, medication dispensing, or any specific treatment outcome.

State Eligibility Requirements

Telehealth services are available only to patients who are physically located in states where Invigor8 Medical Care providers are properly licensed and authorized to practice. Patients may be required to verify their identity, physical location, and medical history prior to receiving care.

Providing inaccurate, incomplete, or misleading information may result in denial, suspension, or discontinuation of services.

Prescription and Pharmacy Services

If treatment is determined to be medically appropriate, prescriptions may be transmitted to affiliated licensed pharmacies, including Optio Rx and Boom Rx. These pharmacies operate independently and are solely responsible for prescription fulfillment, compounding, dispensing, shipping, and compliance with applicable pharmacy regulations. Invigor8 Medical Care does not directly manufacture, compound, or dispense medications.

Compounded Medications Disclosure

Some prescribed medications may be compounded

medications prepared by licensed pharmacies. Compounded medications are not FDA-approved, may differ from commercially manufactured products, and are prescribed only when deemed medically appropriate based on the evaluating provider’s clinical judgment. Patients are encouraged to discuss all potential risks, benefits, side effects, and available treatment alternatives with their healthcare provider prior to beginning treatment.

Telehealth Risks and Limitations

By participating in telehealth services, patients acknowledge and understand that remote healthcare may have limitations compared to in-person medical evaluations, including the inability to perform certain physical examinations, potential technical or connectivity issues, incomplete transmission of information, and limitations in emergency response capabilities. Patients further acknowledge that telehealth services may not be appropriate for all medical conditions, symptoms, or clinical situations.

Emergency Medical Situations

Invigor8 Medical Care does not provide emergency medical

services. Patients experiencing chest pain, shortness of breath, severe allergic reactions, suicidal thoughts, loss of consciousness, or any other medical emergency should immediately call 911 or seek emergency medical attention from the nearest emergency care provider.

Privacy and HIPAA

Invigor8 Medical Care takes reasonable measures to protect patient privacy and maintain compliance with applicable HIPAA and healthcare privacy regulations.

However, no electronic communication system can be guaranteed to be completely secure, and patients acknowledge the inherent risks associated with electronic transmission of health information.

Patient Responsibilities

Patients agree to provide accurate and complete medical information, maintain updated medication and health histories, follow provider recommendations, use prescribed medications only as directed, and promptly report any side effects, adverse reactions, or concerning symptoms. Failure to comply with treatment recommendations or provide accurate information may impact treatment eligibility, continuity of care, and overall treatment outcomes.

No Guarantee of Results

Individual outcomes and treatment responses vary significantly. Invigor8 Medical Care does not guarantee weight loss results, symptom improvement, cosmetic outcomes, or any specific medical benefit. Treatment success depends on

numerous individual factors, including medical history,

adherence to provider recommendations, nutrition, exercise, lifestyle habits, and overall patient participation in the treatment plan.

Consent to Telehealth Services

By using Invigor8 Medical Care services, patients voluntarily consent to receive healthcare services through telehealth technologies and acknowledge their understanding of the potential benefits, limitations, risks, privacy considerations, and available alternative treatment options associated with telehealth care. Patients may withdraw consent for telehealth services at any time by contacting the support team.
